Kanker Payudara dalam Kehamilan (Breast Cancer in Pregnancy)
ABSTRACT
Breast cancer is the most frequent cancer and cause of cancer death in women, and is also the most common cancer in pregnant and lactating women, occurs in 1 of every 3,000 pregnancies, Breast changes during pregnancy make clinical assessment difficult. Cancers are usually detected at later stage. Ultrasound and a mammogram can be performed with little risk to the fetus. Biopsy is important for diagnosis. In the first trimester of the pregnancy modified radical mastectomy is treatment of choice. Conservative surgery with radiation therapy has been used for breast preservation. Adjuvant chemotherapy and radiation therapy should be avoided during the first trimester due to the risk of teratogenicity. Chemotherapy may be given after the first trimester. Breast feeding should be stopped before surgery to make the breast smaller and lessen the blood flow. It should also be stopped if chemotherapy is planned. Six cases were reported in two years at dr. M. Djamil Hospital Padang. All cases were in late stage. One patient has lung metastasis. Modified radical mastectomy was performed in five patients. Two patients had adjuvant chemotherapy post partum. One patient had premature labour. One patient came first day after labour, and mastectomy was performed on seventh day post partum.
